  2. Print a copy of the completed form.
  3. Click the "Submit" button at the bottom of the form.
  4. Write a check, drawn on a US bank, for the total amount of your purchase, including shipping. Make the check payable to Bolivia Web
  5. Send the check and the printed copy of your order form to our mailing address:

Allow up to 2 weeks (10 business days) for delivery of merchandise mailed in the US starting the day after we receive your payment. Allow up to 3 weeks (15 business days) for delivery of merchandise mailed abroad starting the day after we receive your payment.
